Turning 65 Medicare Checklist

Hitting your 65th birthday is a major milestone, but it also comes with a mailbox full of insurance junk mail and confusing enrollment rules. If you’re feeling overwhelmed by all the choices, you aren't alone!

Whether you plan to retire right away or keep working, here is your step-by-step checklist to navigate Medicare smoothly—and protect your hard-earned savings.


1. Know Your 7-Month Initial Enrollment Window

When you turn 65, you qualify for your Initial Enrollment Period (IEP). This is a critical 7-month window to sign up for Medicare without paying lifetime late-enrollment penalties:

  • 3 months before your 65th birthday month

  • Your birth month

  • 3 months after your 65th birthday month

💡 Pro Tip: Don't wait until the last minute! Setting up your coverage 1 to 3 months before your birth month ensures your card arrives on time and there’s no gap in your doctor visits or prescription coverage.

2. Understand the Medicare Basics (Part A & Part B)

Before looking at private insurance options, you need to know what Original Medicare covers:

  • Medicare Part A (Hospital Insurance): Covers inpatient hospital stays, skilled nursing facility care, and hospice. For most seniors, Part A is premium-free if you’ve worked 10 years (40 quarters) paying Medicare taxes.

  • Medicare Part B (Medical Insurance): Covers doctor visits, outpatient care, lab tests, and preventive screenings. Part B carries a monthly premium set by the government.


3. Decide: Medicare Advantage vs. Medicare Supplement (Medigap)

Original Medicare only pays about 80% of your covered medical costs, leaving you with an unlimited 20% out-of-pocket gap. To protect your wallet, you have two main routes to close that gap:

Option A: Medicare Supplement (Medigap) + Part D

  • Freedom to visit any doctor or hospital nationwide that accepts Medicare (no network restrictions).

  • Predictable out-of-pocket costs with minimal to no copays when you receive care.

  • Paired with a standalone Part D plan for prescription drugs.

Option B: Medicare Advantage (Part C)

  • All-in-one plans offered by private insurance companies that replace Original Medicare.

  • Usually feature $0 or low monthly premiums and include built-in prescription drug coverage.

  • Often include added perks like routine dental, vision, and fitness memberships, but operate within local HMO/PPO doctor networks.


4. Are You Still Working Past 65? Check Your Options

If you or your spouse plan to keep working past 65 and have group health insurance through an employer, you might not need to sign up for Medicare Part B right away.

However, this depends heavily on the size of your employer (under or over 20 employees). Before delaying Part B, let’s review your group coverage together to ensure you won't trigger a late-enrollment penalty down the road.


5. Don't Forget Essential Extra Coverage

Original Medicare does not cover routine dental cleanings, eyeglasses, hearing aids, or cash payouts for extended hospital stays. During your review, I can help you look at affordable supplemental coverage like:
